move behind your frame
using your magic wand click inside the heart frame
re size to your liking
add tube of your choice I'm using keith Garvey
copy paste as a new layer your tube again re size again
erase off so your head is over the frame
copy and paste element 27
image re size to your liking move behind your first tube
erase off access
add a drop shadow
copy paste element 2o
image re size by 70
add a drop shadow
layers duplicate
move it to the other side
copy paste element 32
image re size by 70 percent place on the left
copy paste as a new layer element 4

place at the bottom
